Geriatric Services

ReliableCare is a NY state approved home care agency servicing established to serve your medical needs at home.
Perhaps you need a few hours of help a day to assist with bathing or dressing, light housekeeping, or companionship. Or, you may need a full day of service while your loved ones are out of the home, to ensure your safety. If you are an adult child caring for an elderly parent, especially if you live a distance from them, you need to have experienced, quality care.
Who will you call if you want to remain in your own home, but you cannot be there alone? You can call ReliableCare, and know that we will help you decide what type of services you need and for how long.
What happens when you begin services? A Registered Nurse will come to your home and conduct a physical and safety assessment, giving you recommendations to keep your home safe. All of our nurses are experienced with the needs of the geriatric population. If a Home Health Aide is needed, the nurse will discuss a care plan with you, your loved ones and the aide, and orient the aide to your specific care plan. After services begin, the Registered Nurse will make periodic home visits to reassess your needs, discuss how the care plan is meeting your needs, and supervise the Home Health Aide. A nurse is available 24 hours/7 days a week.
Our live-in aides are certified aides who have been chosen to provide this unique and much needed service. It has been shown that most people would choose to remain in their home rather than be in a nursing home, and our live-in service provides that alternative for you or your family member, most times at a cost significantly less than a nursing home.
All of our paraprofessional staff undergo a background and fingerprint check.

Every patient receives an individualized treatment plan to ensure that we are meeting his or her needs. After conducting our own assessment of the patient’s physical, psychological, and social state, we will work closely with physicians, family members, friends, and the community to make sure that our patient gets the right care.

Pediatric Services

At ReliableCare, we understand that your child’s health is most important. That’s why we offer pediatric home care for you and your children. If your child needs skilled home care, ReliableCare can offer you RNs and LPNs who are trained and supervised to provide care under the direction of your child’s physician.
We work with most insurance companies, and can also make arrangements for out-of-network on an individual case basis. ReliableCare provides services that cover all ages, from pediatric home care to senior home care to assure that you’re getting the best care you and your family deserve.

The live-in aide can eventually become like another member of the family, respecting your individual cultural or religious preferences. All of our live-in aides are provided with transportation to your home, and when a switch is made for a break for the aide to go home, ReliableCare provides the relief aide, transports him/her to the home, and a Registered Nursing Supervisor will orient the relief aide to the care plan that you and your family have approved.

Rehabilitation

Our highly skilled rehabilitation team includes Physical Therapists, Occupational Therapists, Speech and Language Therapists and Medical Social Workers. Many times, a discharge from the hospital or a rehabilitation facility, is not the end of your rehabilitation needs. The need for rehabilitation can extend to the home, and continues your progress in your own, familiar surroundings. Our therapists conduct a safety assessment and make recommendations to your physician if there is equipment that would aide in your recovery. Our therapists come to your home and following your physicians orders, continue to work with you to attain the best possible outcome.
